Chastity belts are common I recommend you check out http://www.extremerestraints.com/ and http://www.bdsmtoybox.com/
BDSM toy box has an attachment device that you can put on almost any belt to hold a dildo or buttplug as for adding spikes all you need to do is buy a leather hole punch and some screw spikes if you can't find them loose try buying some goth spike gear.
